Minimum Qualifications
Bachelor's degree in community health, nursing, education, communications, social or behavioral science, communications, or a related field and two years professional work experience
Preferred Qualifications
Preference will be given to applicants who have excellent organizational and communication skills; group presentation and training skills; conducting health promotion programs; and working with healthcare systems and electronic health records.

Summary of Work
This position uses a team approach to implement and advance hypertension management strategies in health care systems and other settings across the state. Duties include:
Provide education to health care providers on evidence-based strategies to identify and manage patients with hypertension, including the use of health information technology.
Develop partnerships with other health care providers, including but not limited to chiropractors and dentists, to advance hypertension identification and referral to appropriate treatment.
Develop a self-measured hypertension monitoring program.
Maintain partnerships with coalitions, task forces and stakeholders.
Provide training and technical assistance to partner organizations, including but not limited to grant applications, work plans, budgets and progress reports.
Assist with completion of grants, publications and oral and written reports.
Evaluate grant programs and make recommendations for improvements.
